We work collaboratively with healthcare professionals, support coordinators, aged care providers and community organisations to deliver personalised, evidence-based support.

2. Referral Review

The P4 Movement team will be in touch within 48 hours if the referral is appropriate for our service or requested to provide more information to assess suitability.

3. Book an appointment

One of our friendly team will be in touch with the client to schedule an initial assessment with one of our experienced Clinicians.

Eligible for Government support?

If you believe you may be able eligible for Government support through the Support at Home Program or the NDIS, we can connect you with experts who will be able to guide you through the process.
